This case is about a complication of abdominal tuberculosis in the form of a massive lower gastrointestinal (GI) bleed, which was timely intervened by angioembolization. A young man in his mid 20's on empirical anti-tubercular therapy (ATT) for abdominal tuberculosis, presented with severe abdominal pain. The patient then developed frank per rectal bleeding, leading to a significant drop in hemoglobin level, requiring multiple blood transfusions. Upper gastrointestinal endoscopy (UGI) and colonoscopy findings were inconclusive. Contrast-enhanced computed tomography (CECT) of the abdomen was performed, which revealed a contrast extravasation into the jejunum due to a leak in the jejunal branch of the superior mesenteric artery (SMA), followed by selective SMA angiography (digital subtraction angiography), which was arrested by angioembolization. The patient had multiple abdominal lymphadenopathies with omental nodules. Histopathological examination of the omental nodules revealed epithelioid granuloma with Langerhans-type cells. The patient is currently receiving ATT and is doing well.

The edge base bleed type is presented in this paper. Based on the advantages of computational fluid dynamics ‎and the firing range prediction of base bleed projectile, the flight ballistic of base bleed projectile with two kinds ‎of base bleed type is solved by using the computational aerodynamics coupled with particle trajectory, obtained ‎the change laws of operation parameter, flow filed of base bleed projectile with time. The results show that: The ‎performance of the edge base bleed type is better than that of the traditional center base bleed type, and the ‎fire range of the base bleed projectile with edge type is about 2 km higher than that of the center type. The base ‎bleed parameter of the two kinds of base bleed type at the state of drag reduction increase first and then ‎decrease with the increase of time. For the center base bleed type, the drag coefficient increases with time. For ‎the edge base bleed type, the drag coefficient is nearly stable at the reduction state, and is always smaller than ‎that of the center base bleed type. The base flow structure of the edge base bleed type does not change with ‎both the time and base bleed parameter. The base bleed gas always forms the main recirculation zone at the ‎bottom of projectile and plays the role of drag reduction‎‎.

Melanoma is a very aggressive skin cancer that could metastasize to any organ in the body. The treatment of melanomas includes surgical resection, chemotherapy, and immunotherapy. After resections, melanomas could recur at the previous site or present as a distant metastatic lesion. The symptoms of melanoma are vague and primarily occur because of the local disruption of the tissue architecture. Presented here is a case of gastric melanoma that presented with abdominal discomfort and melena in a patient with a history of penile melanoma that was completely resected 3 years earlier. This case illustrates the importance of having metastatic lesions to the intestinal tract as a differential for a patient with gastrointestinal hemorrhage.

Patient A is a 72 year old female referred for urgent upper intestinal endoscopy for a low Hb of 6.0 gm/dL, and a history of iron deficiency anaemia for several years. She received several blood transfusions over a period of nearly 10 years. She gives history of melaena and frequent episodes of epistaxis.

Gastrointestinal function can be compromised in children with a variety of disorders either directly as in Crohn's disease and short bowel syndrome, or indirectly as in cerebral palsy. Whey-based diets have been shown to reduce the degree of regurgitation, gastric emptying times, and gagging in neurologically impaired children. Even an energy-dense (1.5 kcal/mL) whey-based formula showed equally good tolerance and gastric emptying as compared with a regular strength (1 kcal/mL) formula. These diets are well-tolerated and provide nutrition to maintain and achieve growth. Gastrointestinal disorders should be to meet their nutritional needs and promote growth without compromising tolerance. Peptide-based enteral diets are often used as the basis of nutritional therapy for some children with compromised gastrointestinal function. A 100% whey, peptide–based diet containing insoluble and prebiotic fiber was as well tolerated as a commercially available, fiber-free control diet in a small and heterogeneous population of children with impaired gastrointestinal function and was associated with a substantial improvement in stool consistency, especially in neurologically impaired children.
